Medical Insurance is a contract between the policy holder and an insurance carrier. Plans can either reimburse the member for medical expenses, or pay reimbursement directly to the provider. Coverage is usually obtained from either Private Insurance carriers or Government Sponsored Plans such as Medicare. Plans can be purchased on an individual or group basis. The payments or premiums are made monthly to the carrier.
